What is a Missing Receipt Affidavit?

For times when you don’t have or have lost the receipt

A Missing Receipt Affidavit is a document that must be used in all cases where a receipt for a legitimate MIT business expense is both missing and required and it is not possible to obtain a duplicate receipt from the supplier.

Affidavits must be signed by the payee regardless of dollar amount.
